2. The Birth certificate should be issued from the authentic authority of J.N.A.C. / Municipal
Corporation / Panchayat office.
3. The date of birth requests will not be entertained after the admission. Please mention
the original date of birth and the supportive documents are to be produced at the time
of verification.
4. Please fill up all the necessary fields in the registration form.
